If you have a dog or cat, then you should check out MobiPetTags. This company uses Microsoft Tag to create pet tags that encode all your furry companion's vitals. Anyone with a smartphone and the free Tag app can scan the Tag and pull up your pet's identification, medical history, your contact info, and whatever else you want to provide.
You simply fill out the online template, decide which format you want (collar, keychain, travel, wallet), pay up, and then wait for it to arrive in the mail.
